Medical RecordsAs a patient of The Woman’s Clinic, you are afforded clear, legal rights regarding the privacy of your medical records and protected health information. Your privacy is important to us, and we welcome you to read The Woman’s Clinic Notice of Privacy Practices (PDF-English) or Notice of Privacy Practice (PDF-Spanish) in full to better acquaint yourself with how your medical information can and cannot be used. Our Notice of Privacy Practices is available in a take-home brochure that you’ll find at our offices and with new patient packets.
You have the right to inspect and receive a copy of your records (with the exception of certain reasons outlined in our Notice). If you’d like a copy of your medical records, please submit a written request to The Woman’s Clinic Privacy Officer using our medical records release form.
